Melissa Moe, LCSW is committed to protecting the privacy of every client and family. This notice describes how health information may be used and disclosed and how you can access it. I comply with HIPAA and applicable California privacy laws.

Information I collect

To provide care, I may collect your name, contact information, relevant health history, and billing information.

How information is used

Your information is used to provide treatment, coordinate care when you authorize it, and handle payment for services. Records are kept confidential and stored securely.

Limits to confidentiality

Confidentiality is central to therapy. By law, there are limited exceptions where disclosure may be required, including suspected abuse or neglect, a serious threat of harm to self or others, and valid court orders.

Communication

Standard email and web forms are not encrypted and may be vulnerable to unauthorized access. Please do not include sensitive clinical information in email or the contact form. Use these channels for general scheduling and inquiries only.

Your rights

You have the right to request access to your records, request corrections, and receive a copy of this notice. To make a request, contact me at (707) 888-1035.
